Surgery for TMJ

Surgery Is the Last Resort - Not the First Step

The vast majority of TMJ disorders respond well to conservative treatment including night guards, Botox, bite adjustment, and physical therapy. Surgery is reserved for cases where structural joint damage is severe, conservative care has been exhausted, or where imaging reveals pathology that requires surgical intervention.
At Arizona Center for Laser Dentistry in Scottsdale, we always begin with the most conservative effective approach. When surgical evaluation is warranted, we provide detailed imaging, documentation of prior conservative treatment, and a warm referral to oral surgeons and maxillofacial specialists in our network.

Common Questions

TMJ Surgery FAQ

Do most people with TMJ need surgery?
No. The vast majority of TMJ disorders - estimated at 85 to 90 percent - respond to conservative treatment. Surgery is reserved for cases with severe structural damage, failed conservative care, or progressive joint degeneration confirmed on imaging.
TMJ surgery ranges from minimally invasive arthrocentesis and arthroscopy to open joint procedures and total joint replacement. The appropriate type depends on the specific pathology identified on imaging.
A CBCT 3D evaluation is the starting point for determining surgical necessity. Indicators include severe condylar resorption, fixed disc displacement unresponsive to splint therapy, or progressive joint degeneration.
Recovery varies by procedure. Arthrocentesis recovery is typically a few days. Open joint surgery and joint replacement require several weeks of limited jaw function and a structured rehabilitation protocol.
In most cases, yes. Post-surgical splint therapy protects the repaired or replaced joint and helps rehabilitate the bite. We co-manage post-surgical patients to provide appropriate occlusal therapy throughout recovery.
TMJ surgery is typically covered under medical insurance when medically necessary. Documentation of prior conservative treatment and imaging is important for insurance approval. We provide comprehensive documentation as part of the referral process.
